- Full-spectrum primary care. Covered medical services will be billed to Medicare (and Medigap as secondary)

- Have read about the Sozo membership model and the plan you're choosing
- Understand that Sozo is a direct-pay practice. Your monthly membership is paid directly to Sozo, and we do not bill commercial insurance.*
- Understand that membership is month-to-month and billed through your patient portal
