Sozo Lifestyle Medicine offers direct primary care and lifestyle medicine services on a membership and cash-pay basis, with certain services billed to Medicare as required. Membership covers only the services listed in your agreement; it is not insurance and does not replace health insurance.

2. Patient Responsibilities

You agree to:

* Provide accurate and complete health information.
* Pay membership fees and any additional charges as outlined in your membership agreement.
* Use Sozo’s services appropriately and respectfully.

3. Communication

By providing your contact information, you consent to receive communications by phone, email, or text. Text message frequency may vary, and message and data rates may apply. You may opt out of marketing messages at any time.

4. Privacy

Your information is handled according to our Privacy Policy. We do not sell or share your personal data for marketing purposes.

5. Payment & Billing

Membership fees are due as stated in your agreement. Fees are non-refundable except as outlined in your membership agreement or required by law. Additional services, labs, imaging, or referrals may involve separate charges.

6. Limitations of Service

* Membership does not cover hospital care, specialist services, or procedures outside Sozo unless otherwise specified.
* We do not guarantee that services provided will meet insurance reimbursement requirements.
* Emergencies should be directed to 911 or the nearest emergency department.

7. Disclaimers

We provide medical care consistent with our training and licensure, but outcomes cannot be guaranteed. Website and educational content are for informational purposes only and are not a substitute for personalized medical advice.

8. Termination

Either party may terminate services in accordance with the membership agreement. We may decline or discontinue care if necessary, including for non-payment, inappropriate behavior, or relocation outside our service area.
